MRI-guided High Intensity Focused Ultrasound [Brain]: a systematic review

Abstract
Objectives: The aim of this research was to assess the safety and effectiveness of MRI-guided High Intensity Focused Ultrasound (HIFU) in movement disorder.

Methods: The literature review for MRI-guided HIFU was done using 8 national databases including KoreaMed and several international databases including Ovid-MEDLINE, Ovid-EMBASE, and Cochrane Library. The subcommittee and two additional reviewers independently performed each step of the assessment including literature search, application of the inclusion criteria, evaluation of the article quality, and data extraction. The quality of the articles were evaluated using SIGN (Scottish Intercollegiate Guidelines Network), and the level of evidence and the grade of recommendation were carefully determined and documented based on the quality.

Results: Out of 268 articles(excluding 116 duplicated articles) selected, 12 papers were included in final assessment. The safety of the technique is acceptable as the incidence and persistence of complications is similar to the cases reported for the traditional operations, and it reported a decreasing trend in the incidence of postoperative complications according to time. The assessment of effectiveness for MRI-guided HIFU was based on improved clinical symptom, including physical symptoms, mental symptoms and cognitive skills. i) 1 article (controlled study) reported improvement of physical symptoms in the intervention group three months after the procedure, ii) 10 articles (case studies) reported postoperative improvement of physical symptoms as well as mental symptoms, cognitive skills. Quality of life also improved(3 articles).
Conclusion: MRI-guided HIFU is a safe and effective technique for improving the clinical symptoms and quality of life in movement
disorders.
